Sheffield Vision for Neighbourhood Health

Sheffield aims to create a neighbourhood health system that respects the role of citizens and communities.

The current set of NHS reforms include ambitious targets to develop models of neighbourhood healthcare. At the same time communities and neighbourhoods have been developing new models of health and community development. Sometimes it feels like these bottom-up and top-down initiatives are worryingly disconnected.

Sheffield aims to combine both approaches to develop its own powerful approach to shifting power and improving health. The paper below was developed to set out Sheffield's ambitious vision for Neighbourhood Health. It includes 17 design principles which provide real world measures for whether this vision is being achieved.

The paper was discussed and agreed at the Health and Care Partnership Board on the 16th April 2026. Further work on the future governance in Sheffield given the current changes to the organisation of the NHS in South Yorkshire. However all partners agreed that any future of place-based work in Sheffield must build on the foundations of the 147 neighbourhoods and the principles set out in this paper.
