How transition should work for families and young people with disabilities.

The Centre worked with schools and strategic partners in Yorkshire & Humber to develop a radical new model of how transition should work for families and young people with disabilities.

This new model, which has gone on to be adopted by central Government in England, involves giving families budgets for social care, education and health and supporting them to design the best pathway for themselves through school and into adult life.
