Personalised Support Survey deadline 30 June 2016

If you're a service provider working to support people to lead lives as full and active citizens, included in their communities, we need your help.

We're carrying out an international survey to establish how much progress service providers are making. We will use the results of this survey to:

• Publish an independent report outlining progress towards flexible and personalised support
• Share information on what's working and the barriers organisations face
• Start a free network to help organisations to share learning and work together
