This paper provides a clear overview of the Personalised Transition Model as it was first developed in Sheffield and then extended to other localities in Yorkshire & Humber. The model offers a dramatic improvement in the transition experience and is the most far-reaching example of personalisation in action in the UK. This model could not only transform the lives of disabled people and their families, it could also offer a way of improving the post-education experiences of many other young people.
